Donor Recognition Plaque Information

Our donor recognition wall, located in our lobby, serves as a tangible way for our organization to acknowledge and thank the generous individuals, businesses, and foundations that have made significant contributions to help support our programs and our mission.  

As a thank you for your generous donation of $250 or higher, we will create a plaque, with information provided by you.  Your plaque will be displayed on our donor recognition wall that is located in our lobby. 

Each plaque allows for two lines of text with twenty-five characters per each line or up to 50 characters total. Please keep in mind that spaces count the same as characters when completing this form.  You can simply put your name or your company’s name, or personalize as you wish!  

Examples: 

In Memory of Mr. & Mrs. Smith (29 characters) 

We support the Catholic Club (28 characters) 

Jane and Tom Smith support the Catholic Club (43 characters) 

Starfish: $250 

Coral Reef: $500 - $1,000 

Sea Turtle: $2,500 - $5,000 

Shark: $10,000 - $25,000
